Court Bsketball, the CBA-BPE Heat AAU, ABC Inc, facilities used, or anyone associated with this program assume responsibility for accidents and medical or dental expenses incurred as a results of participation in the program. I hold harmless and indemnify, Massasoit CC, the "Basketball PLAYER'S EDGE, Shot Court Basketball, the CBA-BPE Heat AAU, ABC Inc. I waive the right to make claim directly or indirectly for all known and unknown personal injuries or property damage now and hereafter. The applicant is in good health and able to participate fully and is covered by medical insurance as listed above.
REFUNDS : Refunds are pro-rated for injury only, for the "basketball PLAYER'S EDGE, and Shot Court with a Doctor's note, for the remaining sessions from the date written notification is received, minus a $ 25 non-refundable administration fee. There are no refunds for the CBA-BPE Heat AAU.
